The Disabled Children Health Visiting team provide a service for parents/carers of children under five who have a diagnosed disability and have specific health needs that requires specialist health visiting interventions.

  • Assessment of needs
  • Co-ordination of services and managing complex cases
  • Referral and signposting to services/professionals
  • Support around diagnosis
  • Provide advice on disability benefits
  • Assessment, advice and planning around complex sleep problems
  • Assessment, advice and planning around managing challenging complex behaviour
  • Assessment, advice and planning around rigid and restrictive feeding
  • Assessment, advice and planning around Pica and keeping your child safe
  • Assessment, advice, and planning around developing toileting skills
  • Developing independence skills e.g., using utensils for feeding, developing dressing and undressing skills
  • Advice around play and development
  • Advice around the importance of using the correct communication resources to support children e.g. objects of reference, photos, symbols, first and then boards. Utilising social stories to support understanding on areas such as sleep, routine, keeping children safe when going out, supporting toilet training etc
  • Advice and support around dental health, utilising visuals resources to support teeth brushing and referrals to specialist dentist
  • Advice and support about sensory processing difficulties and directing to the occupational therapy sensory processing webinar
  • Advice around keeping your child safe at home and outdoors – early help assessments completed where appropriate for referral to social care occupational therapist for safety assessment
  • Referrals via early help assessment for short breaks service at the Lighthouse. (Integrated disabled children’s services)
  • Provide health reports for education and health care plans (EHCP) when these are requested by the local authority, and we are working with child/parent
  • Providing supervision and advice to universal staff – health visitors, family health practitioners who are currently working with a family and are waiting for our service to be involved

Service Referral Criteria 

The team will accept referrals for children aged under 5 with a diagnosed disability. This includes children with impaired physical, learning, or sensory development, requiring one or more specialist disability health visitor core services. For example, the kinds of conditions that the team support are autism spectrum disorder, downs syndrome, global developmental delay, hearing impairment, visual impairment, cerebral palsy, muscular dystrophy, spinal muscular atrophy, rare genetic syndromes, complex, dual or multiple disabilities etc.  

How to refer to our service
