Hi All,
When referring a patient to the young adults team, there will be 2 different forms:
1. YAT Referral form - New Referrals: This one is for totally new patients, not previously known to our service.
2. YAT Referral form - Transfer of Care: This is for patients who are already known to the service, but are transferring from the community therapists in our team over to the Young Adults Team.
