How do you access our service?
If you are experiencing memory difficulties and would like an assessment, please contact your GP and request a referral to our service. Other healthcare professionals involved in your care may also be able to refer you.
If you receive a diagnosis of dementia through our Memory Assessment Service, you will be offered information about our post-diagnostic support groups during your follow-up appointment.
If you have already received a diagnosis of dementia from another service and would like to attend one of our post-diagnostic groups, please contact us to discuss whether the groups are suitable for your needs. A referral from your GP will be required.
