‘If you/your patient is over 18 and you think you may have an eating disorder, you may be able to obtain a referral to our service:
- Via your GP or other healthcare professional (if they are based in any of the areas on the map below)
- Our referral form for SystmOne users can be found in WAA Community Launch Pad, under WAA Referrals Template (Refer to Eating Disorders)
- Via secondary mental health services (e.g. CMHT) or other organisation (e.g. First Steps, FREED, Beeches).
For all other referrers, a referral form should be completed and returned to us by email to dhcft.eatingdisordersservice@nhs.net or posted to Unity Mill, Derwent Street, Belper, Derbyshire, DE56 1WN.
