Are you over 18 and think you may have an eating disorder? Or is your loved one or patient over 18 and you think they may have an eating disorder? If so, you may be able to obtain a referral to our service:
- Via your GP or other healthcare professional (if they are based in any of the areas on the map below)
- Our referral form for SystmOne users can be found in WAA Community Launch Pad, under WAA Referrals Template (Refer to Eating Disorders)
- Via secondary mental health services (e.g. community mental health team - CMHT) or other affiliated organisations (e.g. First Steps, FREED, Beeches).
For all other referrers, a referral form should be completed and returned to us by email to: dhcft.eatingdisordersservice@nhs.net or posted to: Unity Mill, Derwent Street, Belper, Derbyshire, DE56 1WN.
