Making a referral
Referrals to the service should be made using the standardised referral form here and emailed to Dhcft.perinatalcmht@nhs.net
Things to consider in your referral:
- Does the women or birthing person consent to the referral
- Information about the loss (type of loss, when occurred, information about what happened)
- Prescence of any previous obstetric loss
- Current mental health. Prescence of low mood, sleep disturbance, irritability, reduction in appetite, hopelessness, blame or guilt around loss, anxiety, intrusive thoughts, nightmares, flashbacks.
- Any current medication (antidepressants, mood stabilisers, antipsychotics)
- Any risk to self (self-harm or suicidal thoughts/intent) or others (intrusive thoughts of harm)
- Any safeguarding concerns including if there are other children in the family with safeguarding concerns.
- Does the woman/birthing person require medication review by GP
- Does the women/birthing person require a referral to Crisis Team if expressing thoughts/intent to harm self or others.
- Are you aware if the woman/birthing persons partner would benefit from seeing GP or referral to NHS Talking Therapies
