Young Adult Service

Who are we:

The Young Adult Therapy Service provides a specialist Physiotherapy service to young physically disabled adults aged 16-25 years in the Derby City and South Derbyshire area..  This service is provided to bridge the gap between paediatric and adults services. Our aim is to support you as much as possible throughout your time in the service to promote independence and self management of your disability.  Our team consists of a Senior Physiotherapist and a Physiotherapy Technical Instructor.

How we support you:

  • Work with you to help you to achieve the lifestyle you want by encouraging self management of your health needs.
  • Help to maintain and improve your physical abilities with an exercise programme.
  • Helping you to access leisure/sports facilities.
  • Help you recover after surgery or an injury.
  • Advise on any equipment necessary to help you to become independent.
  • Liaise and refer to other professionals and agencies to ensure you receive appropriate support/treatment, such as referrals to orthotics and occupational therapy.
  • Provide Medical Advice.


How we work:

  •  We are based in the community.We can see you at home, school, college, work or any other agreed place of your choice.

  • We offer clinic appointments at Royal Derby Hospital and community Health Centres.

  • Working as part of a team, we will assess you and provide you with advice and/or treatment to meet your needs.
  • We work closely with other services to help support you in all areas of your lifestyle such as:

                                Independent living




                                Getting out and about

                                Advice on your rights

                                Moving and Handling


Transition Service  (For existing Children under the care of the Community Paediatric Therapy Team)

 If the young person has a physical disability, it may be that they will have on going physiotherapy needs.  When a young person reaches the age of 14, consideration is given as to whether they will require further services after leaving school. This is all part of the transition process.  If this is the case , the Community Paediatric Therapist will refer to the Young Adult Service to start transition.    This transition is a gradual process involving joint working between the Community Paediatric Therapist and the Young Adult Therapists until the child is 16 years old.  At age 16 their care will transfer over to the Young Adult Team who will continue to support until they are 25 years old.
